A DOCS Education member seeks the faculty’s assistance:

I have a 41-year-old female patient who takes 10 mg of doxepin twice daily.

I checked Lexicomp™ for any interactions with diazepam, but it did not list any. Before I proceed with treatment, I would like to confirm it is ok to give diazepam to a patient taking 10 mg of doxepin twice daily. Thank you!

Here is additional info for the patient taking doxepin:

  • Weight: 122 lbs (55.4 kg)
  • Age: 41 years old (female)
  • Baseline Vitals: BP = 109/71 and P = 91 and O2 sat = 99%
  • Only Med: doxepin (10 mg twice daily)
  • ASA II: Her only medical condition is Cramp Fasciculation Syndrome, which is controlled well with the doxepin.

Dr. Anthony Feck, DOCS Education faculty, responds:

 

There is no significant drug interaction between doxepin and DOCS protocol sedation medications. You can use any appropriate DOCS Education sedation protocol with this patient.

You should, however, use local anesthetics with vasoconstrictors judiciously, administering slowly, and monitoring carefully both visually and with a pulse oximeter due to the enhancement of the vasoconstrictor effect with patients taking doxepin.
