I have a patient needing surgical extractions for teeth #2 and #16. The patient is female, age 31, married, taking birth control pills, and has multiple sclerosis. Her medical doctor has approved her for treatment. Are there any special precautions I should take for this patient for oral sedation?

Dr. Anthony Feck, Dean of DOCS Education Faculty responds:

Multiple sclerosis is a progressive continuum of deterioration. In this case it appears the patient is in an early stage of the disease. In the latter stages of the disease, the respiratory muscles can be affected, which is a significant concern when it comes to the CNS depression that accompanies sedation.

If the patient’s doctor has approved her for dental treatment under sedation, then I would proceed as normal with the appropriate DOCS Education sedation protocol, dosing low and slow. Birth control pills can inhibit metabolism of sedation meds, but the interaction is mild.
